The primary concern of a physician is centred\u00a0around the best possible treatment for his or her patients, according\u00a0to patients\u2019 requests and expectations. The healthcare system, in\u00a0contrast, is ideally concerned with the assignment of resources\u00a0in the best interest of society.1 Clinicians, public health physicians,\u00a0economists, commissioners, managers, and politicians need to find\u00a0ways to balance between what is best for the individual patient\u00a0and what society can realistically afford.
